Clinical Care Coordinator-Full time position
We are seeking an experienced registered nurse who is highly motivated with excellent interpersonal skills to join our team at Sydney Southwest Private Hospital, Medical/Surgical ward. The Clinical Care Coordinator is responsible for coordinating safe, timely, and patient‑centred transitions of care from hospital to home or other care settings. You will also be responsible for reviewing and assessing all incoming referrals to determine their appropriateness for admission, ensuring that each patient meets the established clinical, functional, and service‑eligibility criteria. This includes evaluating the patient’s medical condition, psychosocial factors, discharge risks, and overall care needs to support safe and timely admission decision‑making. This role works collaboratively with the multidisciplinary team to identify discharge needs early, plan appropriate post‑acute services, and ensure continuity of care that reduces readmissions and enhances patient outcomes.

Sydney Southwest Private Hospital provides a large range of medical and acute services including 51 bed medical-surgical ward, seven operating theatres, Angiography suite, 10 bed Intensive Care Unit as well as a Day Surgery Unit, Mental Health Unit, Oncology Department as well as pharmacy and radiology services.
As a Clinical Care Coordinator your responsibilities will include:
Patient Assessment & admission coordination
Communication & Liaison
Referral review & Triage
Documentation & Data Management
Quality, Safety & Service improvement
Discharge Planning & Coordination
Supports clinical services as required where activity levels fluctuate
Essential Criteria:
Current Registration with AHPRA
Working with Children check (if relevant)
Specialty nursing experience (desired)
Commitment to Quality Patient Care
Compliance with ACQSHC National standards
Sound clinical decision making and problem solving skills
Excellent communication and customer service skills
Works well independently and within a team
Basic computer skills
