About the role:

An exciting opportunity exists for a suitably qualified Senior Mental Health Clinician to join The Melbourne Clinic's Day Program Therapy Team as a Clinical Lead Discharge Coordinator.

As a valued member of our team, you will provide advanced clinical leadership within the Day Program and oversee discharge planning across our Day Program and Outreach Services. In this pivotal role, you will champion safe and effective transitions from inpatient care to outpatient and community-based settings, while promoting Day Program opportunities and supporting Outreach referrals.

Working closely with patients, families, treating teams and external providers, you will coordinate discharge planning, facilitate referrals, and ensure continuity of care to support positive recovery outcomes. You will play a key role in strengthening connections between inpatient, outpatient and community services, helping patients access the support they need throughout their recovery journey.

The Day Program delivers evidence-based therapeutic programs for individuals experiencing a range of psychiatric conditions within a recovery-focused, multidisciplinary environment. Competitive remuneration will be offered in line with the applicable EBA, based on your classification level and recognised years of experience.

Key Responsibilities: 

  • Drive and promote supportive discharge processes and patient journey from inpatient to outpatient services
  • Provide clinical leadership, support and guidance to Day Program clinicians and Day Program Managers
  • Provide support to facilitators during group therapy sessions, with the potential for group facilitation when the need arises
  • Promote day and outreach programs to secure appropriate outpatient referrals
  • Attend and actively contribute to regular hospital multidisciplinary team meetings
  • Engage diverse internal and external stakeholders to build strong collaborative care networks
  • Lead quality projects for ACHS accreditation and ensure WHS compliance

 
Essential Criteria:

  •  Current AHPRA Division 1 Registration or relevant Social Work/Allied Health/Psychology qualifications
  • Minimum 3 years’ experience with advanced clinical skills in mental health care 
  • Proven experience facilitating group therapy and understanding core group components
  • Ability to guide, support, and foster a positive working environment for a team of experienced multidisciplinary clinicians
  • Sound knowledge of hospital health fund contracts and episode management
  • Exceptional communication skills to collaborate effectively with diverse stakeholder groups
  • Commitment to recovery-oriented, person-centred and quality patient care without judgement
